v15: RTE: Link to Document issues

Bug summary

If you search for a document in "Link to Document":

1. There's no URL below the results, so you have no idea which node it refers to. That's fine if you have one page with this name, but if you have multiple pages with the same name it's not a useful search.
2. The original icon used for the document is not visible, it's always the "document" icon.
