Description

This ticket will be focused on some final bug fixes before production:

In the Map UI, on a provider's entry there is empty space at the bottom of the page. Please fix this.
On the Provider Builder, in the Office Hours section, replace the radio buttons with checkboxes.
Add a title entry for the text component.
On the home page, move the Netlify button at the bottom of the page.
